THE POSITION

The Aging Care Manager 2 position with the Pike County Area Agency on Aging (AAA) offers a chance to make a real difference in the lives of older adults. This role allows you to guide individuals through challenges and support them in staying safe and independent. You will join a team that values compassion, dedication, and strong community service. DESCRIPTION OF WORK

This position focuses on assessing needs, coordinating services, and supporting vulnerable older adults as they work to maintain safety and independence. As an Aging Care Manager 2, you will perform the following duties:

  • Care Planning: Develop informed care plans based on thorough medical, social, psychological, and functional assessments
  • Safety Compliance: Ensure legal and regulatory time frames are met while following all policies, procedures, and statutes
  • Investigative Work: Conduct detailed investigations into situations involving possible abuse, exploitation, abandonment, or self neglect
  • Service Coordination: Complete referrals, arrange needed services, and monitor follow up to maintain service quality
  • Reassessment Tasks: Perform periodic reviews to identify changes in consumer status and adjust support plans accordingly
  • Advisory Support: Provide guidance to staff and the public on legal, medical, and privacy matters affecting protective service consumers

Work Schedule and Additional Information:

  • Full-time employment
  • Work hours are 7:30 AM to 4:00 PM, Monday - Friday, with 30-minute lunch.
  • There are on call requirements with this position.
  • Telework: You will not have the option to telework in this position.
  • Salary: In some cases, the starting salary may be non-negotiable.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS

Minimum Experience and Training Requirements:

  • Six months as an Aging Care Manager 1 or a County Caseworker 1; or
  • Successful completion of the County Social Casework Intern program; or
  • Six months of professional human services or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or
  • An equivalent combination of experience and training.

  • Hiring preference for this vacancy may be given to candidates who live within Pike County. If no eligible candidates who live within Pike County apply for this position, candidates who reside in other counties may be considered.
  • You must be able to perform essential job functions.
