What You'll Do

  • As a vital member of our treatment team, you'll help guide patients through their recovery journey by providing support, coordination, advocacy, and education. Every day offers an opportunity to make a meaningful difference in the lives of those we serve.

Your Impact Will Include:

  • Be the Connection:Collaborate with referral sources, healthcare providers, and community partners to ensure seamless patient care. Participate in daily interdisciplinary team meetings, coordinate follow-up appointments, facilitate Adult Recovery Team meetings, and support urgent patient needs.
  • Build Strong Relationships:Maintain regular communication with patients, families, and treatment professionals, ensuring everyone involved in the recovery process stays informed and supported.
  • Champion Patient Engagement:Encourage patients to actively participate in their treatment journey, including group therapy, treatment planning, and discharge planning activities.
  • Empower Recovery:Connect patients with valuable community resources and support services that promote long-term wellness and successful recovery after discharge.
  • Ensure Compliance and Quality Care:Adhere to program guidelines, treatment protocols, and HIPAA requirements while supporting the highest standards of patient confidentiality and care.
  • Advocate for Patients:Complete excellent documentation to support patients inan appropriate lengthof stay and acceptance to referral Level of Care.

Qualifications

Education: Masters or Doctoral degree in Social Work, Psychology, ProfessionalCounselingor related degree.

Experience: Experience coordinating care, familiarity with community resources, and ability to be self-organizing. Experience in the field of Behavioral Health for at least 2 years is preferred, does not have to be obtained post-Master's

Licensure/Certification:No license isrequired

MINIMUM REQUIREMENTS: Must be certified in CPR upon hire and bi-annually thereafter and must possess or obtain and be able to perform Handle With Care within 30 days of hire and re-certify as needed. Must complete all required mandatory in-services annually. Must possess an Arizona Fingerprint Clearance Card or the ability to obtain. Must be tested for Tuberculosis with a PPD skin test or chest x-ray upon hire and as required thereafter.
