            Position Summary:
            The Licensed Vocational Nurse provides skilled nursing care to residents on the Village campus. This includes direct resident care, conferring with other disciplines, referring to community resources and/or utilizing resource materials as needed. This position participates in case conferences and staff meetings. Completes paperwork in a timely manner and confers and meets with physicians, discharge planners, and other community persons as needed or directed.

            Essential Job Functions:

            Comprehensive Nursing Care:



            • Supervisesand directs care given to residents by LVNs and CNAs, and ensures continuity of care by appropriately directing,coachingand counseling the staff.
            • Coordinates resident care between disciplines.
            • Demonstratesappropriate physicalassessment skills forinitialand follow-up appraisal of resident status.Possesses an adequate knowledge of clinical disease entities.
            • Demonstratesability to perform nursing treatments and procedures as prescribed for resident assigned.Administers those treatments in an appropriate manner.
            • Demonstratesknowledge of specialized diets and medications commonly prescribed to residents and administersappropriate medications.
            • Demonstratesknowledge and awareness of rehabilitative and preventative aspects of care and ensures resident safety, comfort, and protection.
            • Assistsphysicians in making rounds and assessments of residents.
            • AdministerIV therapy within the scope of practice.
            • Evaluatesnursing careby means ofcurrent assessment tools.
            • Reportsdiagnostic data and incorporates it into the plan of care.
            • Assistsin providing resident and family teaching and training.


            Documentation:



            • Providesnotes on each resident reflecting skilled care, resident instruction, and patient/family response. Provides doctornotificationand specificplanfor ongoing care.
            • Submitswritten plans of treatment reflecting resident status and nursing care plans ina timelyandaccuratemanner.
            • Submitsrequired documentation which consistently meets guidelines within thetime framesestablishedby the facility.
            • Followsdocumentation procedures as required by Title XXII and OBRA rules and regulations.Usesaccuratemedical terminology.


            Personal and Professional Development:



            • Attendsworkshops, seminars, or classes to further develop professional knowledge/skills byseekingor accepting new or challenging assignments whenrequested.
            • Establishesprofessional goals in coordination with nursing supervisor and shows progress towards achievement.
            • Participatesin orientation programs for new nursing employees, students or selected individualsdesignatedby the facility.
            • Participateson internal committees/projects when requested andparticipatesin staff meetings on a regular basis.


            Case Management:



            • Communicatesinformationregardingresidents at team meetings, and coordinates care provided to the residents.
            • Establishesshort- and long-termgoals for total resident care and management.

            Starting pay ranges from $40.00 - $44.44 per hour as an on-call position. Factors such as scope, responsibilities, experience, education, job-related skills, internal equity, and market considerations may influence base pay.

            Qualifications:
            Education:



            • Valid California Licensed Vocational Nursingcertificate.
            • CPR certification.







                          Training and Experience:



                          • One (1) year recent skilled nursing experience preferred.


                          Knowledge and Abilities:



                          • Possesses an in-depth knowledge of comprehensive nursing care which includes clinical disease entities, the physical assessment skillsrequiredforinitialand follow-up appraisal on all assigned residents, and know the treatments, specialized diets, and preventative aspects concerning each resident.
                          • Possessesadditionalknowledge of direct resident care services which ensures the safety,comfortand protection of residents.
                          • Has the ability to perform tasksindicatedfor diseases prevention and restorative measures, ability to document professional nursing services provided with a realistic plan for reaching short term andlong-rangegoals, and able to make decisionsregardingsafety needs of residents including adequate equipment.
