Summary: This position supports UNC Health's Community Health and Engagement (CHE) department outreach efforts by advancing access to care through culturally responsive engagement, language access support, patient navigation, resource coordination, and health education. The Outreach and Access Specialist will build and sustain strategic partnerships in priority focus areas to improve continuity of care coordination and access across the statewide health care system. This role supports clinicians with vaccination clinics, biometric screenings, and data entry at outreach events and communitybased clinics. The position collaborates closely with department leadership and other partners to promote accessible health services for patients and community members. Ideal candidates will bring bilingual (Spanish) capabilities and experience working within a healthcare system to effectively support diverse communities and care coordination efforts. Responsibilities: 1. Supports community outreach, including but not limited to, mobile health clinics community health fairs and festivals, community forums, and other community events. Responsibilities include planning, coordination, and execution of outreach events. 2. Supports community-based clinics, CHE Mobile Health clinics, and telehealth visits, including interpretation services as needed, and documentation in the EPIC electronic health record. 3. Provides navigation support to connect patients and community residents to health services and social drivers of health (SDOH) resources. 4.Maintains current knowledge of health and wellness trends and UNC Health programs and services, by attending workshops, in-service training, and actively participating in UNC Health community events. 5.Educates community members on health and wellness topics on behalf of UNC Health and provides evidence-based health education materials. 6.Supports programs and events related to Juntos Podemos, Joint Commission Language Access standards, the Commission on Language Access accreditation standards, or other language access programs. This may include performing document translations and language interpretations on an as needed basis to increase accessibility for non-English- speaking populations. 7.Ensures consistent and accurate program data and activity tracking, including development of narrative reports, presentations, tables and graphs. 8. Performs other duties as assigned or requested.

Other information: Education Requirements: * Bachelor's degree from four-year college or university preferably in a related field Licensure/Certification Requirements: * Requires valid driver's license. Professional Experience Requirements: * Minimum of three (3) years of experience in community outreach, community health, public health, language access services, program administrative support, or related field. Knowledge/Skills/and Abilities Requirements: * Demonstrated computer skills particularly with Microsoft Office, PowerPoint, word processing and database applications and knowledge of standard office equipment. * Ability to work and communicate with a wide variety of internal and external clients and customers in a polite, courteous, and cooperative manner. * Excellent oral and written communications skills, especially in public speaking. * Demonstrates responsible behavior and attention to detail. * Able to effectively organize and present information in group settings, both formal and informal. * Must be able to work flexible hours, including evenings and weekends if necessary and able to travel to locations in the community as designated. * Display a high level of initiative, effort and commitment towards completing assignments efficiently. * Initiative to work with minimal supervision. * Spanish bilingual preferred.
