LOCATION OF (1) POSITION(S) TO BE FILLED: DEPARTMENT OF DISABILITY AND AGING (DDA), ADMINISTRATION DIVISION, DAVIDSON COUNTY

A Motor Vehicle Records screening will be conducted prior to employment.

This position is designed as Hybrid (In office and Remote).

This position requires a criminal background check. Therefore, you may be required to provide information about your criminal history in order to be considered for this position.

Qualifications

Education and Experience: Graduation from an accredited college or university with a bachelor's degree in special education, education, rehabilitation or therapeutic health, nursing, sociology, social work, or any behavioral science field and experience equivalent to two years of working with persons with disabilities, aging, and/or mental health work.

Substitution of Education for Experience: Additional graduate coursework in special education, rehabilitation or therapeutic health, nursing, sociology, social work, or any behavioral science field may be substituted for the required experience on a year-for-year basis.

Substitution of Experience for Education: Qualifying professional intellectual and/or developmental disabilities, aging, or mental health work may substitute for the required education on a year-for-year basis.

OR

Education and Experience: Graduation from an accredited college or university with a bachelor's degree and experience equivalent to three years of working with persons with disabilities, aging, and/or mental health work.

OR

Education and Experience: Graduation from an accredited college or university with a bachelor's degree and completion of an internship with the Department of Disability and Aging.

Necessary Special Qualifications:
1. Complete a background check in a manner approved by the appointing authority.
2. Some positions may require a valid driver's license.
3. A Certified Work Incentives Coordinator (CWIC), Community Partner Work Incentives Counselor (CPWIC), or Certified Work Incentive Practitioner (WIP-C) Certification may be required for some positions.

Experience can substitute for degree.

Overview

The Provider Support Specialist position will primarily work with other DDA representatives to facilitate administrative responsibilities relative to legislation for a residential pilot program for adults with intellectual and developmental disabilities. This position may also complete tasks associated with provider support activities for contracted providers such as remediation, individual follow-ups, wellness reviews, mandated technical assistance, new provider on-boarding, etc.

Responsibilities

  1. Facilitate the development of the required experience assessment/survey used to support the legislation, if applicable.
  2. Oversee administration of the experience assessment/survey, if applicable.
  3. Complete data management tasks such as data entry and data analysis needed to identify trends or individual issues requiring remediation, complete various monthly, quarterly, or as needed reports, report on outcomes of the pilot program in an annual report required by legislation.
  4. Oversee and facilitate provider supports to those living on the residential campus.
  5. Conduct intermittent on-site reviews, as needed
  6. Provider Supports, function as the provider supports liaison to the regional offices, data management and reporting, identify and implement improvement to provider support processes and procedures in collaboration with internal and external stakeholder. Assist with Identification, development, implementation, and revise as needed, of written policies and procedures.
  7. Facilitate and/or participate in various meetings, work groups and committees as directed to problem solve, review provider performance issues/concerns and/or work through policy and procedure.
  8. Assist customers with provider support inquiries.
