Job Summary:

The LPN Staff Coordinator functions as a Licensed Practical Nurse primarily on weekends, coordinating home health activities, triaging calls, and completing home visits indicated by patient need. The Coordinator supports Home Health leadership with weekend census needs and ensures compliance with privacy regulations.

Essential Functions & Responsibilities:

  • Receives and enters patient referral information into the EMR.

  • Performs home visits and nursing tasks allowed for LPNs. Provides patient and caregiver instruction based on established care plans.

  • Supports delivery of quality, compliant home health care.

  • Conducts patient rounding for assigned patients and reports updates to the care team.

  • Assists in resolving care issues within LPN scope and escalates when necessary.

  • Contributes to the nursing process by collecting data, implementing care, and reporting patient responses under RN supervision.

  • Participates in performance improvement efforts.

  • Performs other duties as assigned.

Qualifications:

  • Valid RN License from Virginia or reciprocal compact state required.
  • High school diploma or equivalent required.

  • Valid LPN License from Virginia or reciprocal compact state required.

  • AHA BLS Provider CPR required at hire.

  • One year recent medical/surgical nursing experience required.One to two years' recent home health experience preferred.

  • Validdriver'slicense and must meet MWHC Driver's Suitability criteria.

Physical Requirements: Constant (67-100% of workday) use of arms and hands; frequent (34-66% of workday) standing, walking, and sitting; occasional (0-33% of workday) bending, stooping, and squatting; ability to lift 35 lbs.; ability to push and pull up to 20 lbs.; auditory and visual skills to include color determination.

Mental Requirements: Possesses critical thinking and analytical skills. Ability to multi-task. Ability to communicate effectively and collaborate with a multi-disciplinary team. Capacity to cope with difficult situations. Ability to tolerate irregular hours including evenings, nights, and weekends.

Environmental Requirements and Exposure Hazards: Potential risk of exposure to radiation and toxic chemicals. Potential for exposure to bloodborne pathogens; must be able to wear appropriate personal protective equipment.
