FTE: _1.0, Shift: _Day, Schedule: _8-4:30pm In the PACE program, Occupational Therapists support participants' safety, independence, and quality of life through home visits, where they assess the environment, recommend adaptive equipment, and implement strategies that promote aging in place. They also provide case management, monitoring functional changes, updating care plans, and coordinating needed services. As members of the Interdisciplinary Team (IDT), OTs collaborate closely with nursing, therapy, social work, transportation, primary care, and other disciplines to create comprehensive, participant-centered care plans. They contribute vital functional insights that help the IDT make informed decisions and ensure coordinated, holistic care. Position Summary The Occupational Therapist is an exciting opportunity to administer evaluations and therapeutic procedures for patients in the home health or PACE setting. This role offers you the capacity to apply your initiative, analysis and service orientation toward the support of your patients every day.Responsibilities

As required by various pay transparency laws, we share a competitive range of compensation for candidates hired into each position. The pay scale is $99,341.00 - $150,571.00 USD. However, pay is influenced by factors specific to applicants, including but not limited to: skill set, level of experience, and certification(s) and/or education.
