Under the direction of the primary care operations team, the Primary Care Nurse works directly with Mercy Primary Care Providers to provide safe, timely, effective, efficient, equitable, patient-centered care in a systematic way. The Primary Care Nurse will partner with patients, providers, and other members of the clinical care team to drive quality outcomes for an attributed panel.
Position Details:
Overview: The Primary Care Nurse completes triage and identifies abnormal patient symptoms, laboratory findings and notifies physician or provides treatment according to standard Primary Care developed protocols. Assists with EPIC in-basket management including, but not limited to incoming and outgoing telephonic or My Mercy messages, test results, medication refill requests, etc. Inputs orders as needed to aid provider and clinical care team.
