Job Description

Elwyn seeks a Care Manager Extender to play a vital role in supporting care managers by assisting with care coordination, outreach, and engagement activities. This position helps ensure that individuals with behavioral health conditions receive timely, effective, and person-centered services. The role emphasizes collaboration, health promotion, and resource navigation to improve outcomes for members.

ESSENTIAL DUTIES:

  • Conduct outreach, engagement, and follow-up with members to support care management goals.
  • Coordinate services and appointments, including wellness reminders and transportation arrangements.
  • Participate in health promotion and education activities.
  • Share relevant information with care managers and care team members to support care planning.
  • Track and provide referrals, and assist individuals in accessing community-based resources and social supports.
  • Attend meetings related to care planning and provider engagement.
  • Assist care managers in identifying and addressing unmet health-related resource needs.
  • Integrate the PRISM Model for effective treatment of serious and persistent mental illness into daily practice.
  • Complete all required training related to care management and tailored care plans.

QUALIFICATIONS:

  • Must be at least 18 years of age.
  • High school diploma or equivalent.
  • Lived experience with a behavioral health condition and certification as a North Carolina Certified Peer Support Specialist. OR
  • Two years of paid experience performing the essential duties listed above, with at least one year working directly with individuals with behavioral health conditions.
  • Must be able to work in a HIPAA-compliant location if telecommuting.
  • Must maintain any required certifications as applicable.
