The purpose of the Case Manager Assistant is to support and under supervision of the Clinical Case Manager to coordinate referrals and implement in facilitating the discharge plan developed and identified by the Clinical Case Manager. This position efficiently and effectively coordinating movement through the continuum of care to ensure timely discharge by effectively coordination of necessary durable medical equipment (DME), home care services, home health nurse/aide, therapies, hospice, home infusion, enterals and teaches.
The CMA will compile documentation from the patient's chart through Epic to produce a discharge packet for all patients discharging to a SNF/Acute/Rehab facility this packet will be delivered to the patient's floor before the arrival of the ambulance. The CMA will monitor all primary and secondary Medicare in-patients insuring they are delivered the Important Message from Medicare before discharge. The CMA will reach hospital authorizations approvals/denials and requests for additional medical documentation. Verify correct level of care in the Clinical Case Management module and Epic Auth Certification tab.

Minimum Qualifications - For full consideration, applicants are encouraged to upload license and/or certification if required of the position
- High School Diploma, GED or equivalent work-related experience
- Two years (2) experience in acute or post-acute discharge planning or case management experience in a large academic healthcare setting.
- Extensive knowledge of health insurance regulations, policies and procedures, Medi-Cal, Medicare guidelines, private insurance and managed care operations and workers compensation.
- Knowledge of terminology, abbreviations and acronyms of medical and nursing treatment plans and level of care.
- Physical and emotional stamina to regularly work within tight time frames and critical deadlines; to deal with upset irate or hostile members of the public.
- Good problem solver with the ability to define, clarify, and set priorities for producing quality results.
- Must have the ability to learn the dynamics of the organization, both operational and political, and must be resourceful and diplomatic in facilitating the completion of projects.
- Knowledge of use of computers, including Epic and Allscripts applications, WEB based search engines, and other software programs, including Microsoft Office, i.e. PowerPoint, Excel and Access Typing skills and working knowledge of printers, copies, scanners and fax machines.
- Understands the need to utilize discretion and maintain confidentiality when discussing patient matters.
- Ability to read and comprehend English text.
- Must be able to communicate effectively with diverse audiences including physicians, nurses, allied health professionals, administrators and a wide variety of non-technical staff.
- Ability to request authorizations electronically for outpatient professional medical services via external websites/web-links.
Preferred Qualifications
- Bachelor's degree in Healthcare Management, Public Health, or other healthcare or related field highly preferred.
- Minimum two years (2) experience working in a health care organization
- Demonstrated awareness of and sensitivity to dealings with cultural and socioeconomic diversity.
- Knowledge and understanding of institutional goals and principles.
- Knowledge of DME/HH resources in the community and UCDH.
