The Structural Heart Nurse Navigator is a key member of the multidisciplinary heart team and serves as a clinical expert, patient advocate, educator, coordinator, and liaison for all structural heart patients and their respective family members. Serves as the primary patient resource in the preoperative and postoperative outpatient phase of care including education, planning needs, data management and collection, and process improvement. Responsible for ensuring all patients navigating within the structural heart program receive quality and comprehensive services. Collaborates with the team on patient care throughout the continuum in collaboration with the multidisciplinary team. Serves as a liaison for team members to include physicians, midlevel providers, RNs, and other department staffmembers. Provides resources and assistance with accessing clinical and supportive care services offered within the Structural Heart Program and within MLH and the community.

Position: Nurse Navigator, Heart Program
Shift: Full time - 40 hours a week - Monday-Friday, no weekends or holidays!

Experience:
Minimum 2 years' Cardiovascular inpatient or outpatient experience.
Cardiac Surgery or Critical Care hospital experience.
Preferred Knowledge and experience of structural heart program.
Preferred 2 Case Management or Patient Navigator experience in hospital or clinical setting.

Education:
Minimum 4 Year / bachelor's degree BSN
Preferred Graduate Degree MSN

Licensures/Certifications:

Current license to practice nursing in the Commonwealth of Pennsylvania.

Maintains CPR and ACLS certification.

BLS Certification by American Heart Association preferred upon hire, required in timeframe specified in CPR (BLS)/ALS/PALS Policy.
