Intake and Referral Program Coordinator (Comprehensive Gender Care)

The M Health Fairview Comprehensive Gender Care Program brings together specialists from across our system dedicated to providing compassionate and coordinated care for transgender, non-binary, and gender-diverse patients. We're home to an interdisciplinary team of experts - ranging from primary care providers to mental health specialists and gender-affirming surgeons.

• Provides leadership and overall coordination to accomplish the implementation of special projects.
• Works with team members to establish clear, precise implementation plan, including goals, objectives and timeframes.
• Actively participates in creating and implementing improvements.
• Partners with Surgery Service Line Leadership to support strategy deployment activities.
• Assists with Performance Dimension leaders as needed.
• Supports leadership by providing onsite support, direction, and backup to Comprehensive Gender Care team.
• Assists in planning, coordinating and facilitating of meetings; prepares and distributes materials necessary for meetings; attend specified meetings and takes minutes to ensure accurate records of meeting are available.
• Acts as a liaison for the Program Leadership between internal and external customers.

• Bachelor's degree in related field
• 2+ years of office experience with an emphasis on project management
• Experience with gender care programs and populations

Location: M Health Fairview Clinic and Surgery Center - 909 Fulton St. SE Minneapolis, MN 55455

Hours: 40 hours / week (8:00 AM - 4:30 PM or 9:00 AM - 5:30 PM with 2-4 days onsite)
