Overview: Community Support Specialist - Youth and Family Support Services

This full-time position will join the Heritage Youth and Family Support Services team to provide support, engagement, and linkage to various resources for youth and families receiving treatment in our mental health programs. Services will be provided in array of settings including individual's home, community-settings, community agencies, schools, and Heritage.

Core Responsibilities Include:

  1. Provide case management and community support services and manage a caseload.
  2. Assist youth with developing and practicing coping skills to manage mental health, substance use, and/or health-related symptoms
  3. Assistance with developing positive supportive activities, identifying and cultivating natural supports (relationships with family members, peers, wellness activities, support groups, or identifying/participating in positive social activities that support recovery).
  4. Provide case management services (referral, linkage, and advocacy) in the home, community, and providers sites including (but not limited too): housing/shelter, food, seasonally appropriate clothing and other basic needs; entitlements and financial management; medical; socialization, vocation/education; and transportation.
  5. Telephone and face-to-face engagement of youth and families who have been referred to community-based mental health treatment
  6. Participate in a team-based approach to providing treatment, including collaboration with family, clinicians, nursing, healthcare providers, probation, and other partners invested in the youth's recovery

Knowledge, Skills and Abilities:

  1. Ability to meet people where they are at in their recovery, non-judgmental, and ability to build rapport quickly;
  2. Ability to work as in a team based environment;
  3. Ability to tolerate ambiguity, uncertainty, and change;
  4. Ability to navigate electronic health records;
  5. Ability to respond to supervisors, person served, and other clinical staff providing services to the person served.

Education and Experience:

Candidates are required to have designation as a Mental Health Professional (MHP). A Mental Health Professional (MHP) is defined as 5 years supervised experience in behavioral health, Certified Recovery Support Specialist (CRSS), or a Bachelor's Degree in psychology, social work, or human services.

Valid drivers license, reliable means of transportation, and proof of current automobile insurance are required.

Salary Range:

Mental Health Professional: $23.00-$26.00 per hour - dependent on education and experience
