Advanced Practice Provider - APRN

Hourly Pay Range:

Exciting opportunity to join our palliative care team. The APP will practice along the continuum of care to support palliative care patients in the inpatient and outpatient setting.

Job Description:

Under the direction of a physician, the APP provides palliative care services to patients. Obtains detailed patient histories and performs physical examinations, orders imaging, lab tests and diagnostic procedures and interprets results. Instructs patients on the prescribed therapeutic regimen. Maintains records and writes progress notes on status, treatments and procedures. Follows patients through their course of treatment in a clinic and hospital setting. Collaborates with physicians and the health care team to achieve optimal patient outcomes. Develops a comprehensive plan of care that includes interventions and treatments. Prescribes pharmacologic and non-pharmacologic therapies as appropriate.

Demonstrates evidence-based practice. Participates in the development, implementation, and evaluation of standards of practice in the specialty. Provides staff education as indicated. Assesses the learning needs of patients and families across the continuum. Employs strategies, interventions and teaching to promote, maintain, and improve health. Participates in the development, implementation, and evaluation of service line goals. Supports the strategic plan of the organization. Adheres to all Endeavor Health policies and procedures.

• Location: Naperville, Elmhurst, Plainfield
• Full time.
• Hours: Monday-Friday. Option to work four, ten-hour days or five, eight-hour days.

• Current IL license.
• Current DEA certification.
• Current CPR certification.
