Job Summary

The Loudoun County Health Department is seeking a skilled, independent, and highly motivated Registered Nurse to join our growing Long-Term Services Support (LTSS) program. This position is based at our office in Ashburn, Virginia, with core work hours of Monday-Friday, 8:30 a.m. to 5:00 p.m. Registered Nurses in this role will provide screenings in residents' homes to determine eligibility of residents requesting services through the Commonwealth Coordinated Care Plus (CCC+) Waiver program. Additionally, this team will support the Tuberculosis program located in Ashburn. A structured preceptorship is provided to ensure a strong foundation for success.

Responsibilities include, but are not limited to:

  • Providing comprehensive pre-admission health screenings for the CCC+ Medicaid waiver program for adults and children with medical and nursing needs in the client's home.
  • Educating clients about health information and disease prevention.
  • Making referrals to other agencies on behalf of the client.
  • Serving as a resource and subject matter expert to the public, local providers, and schools.
  • Conducting TB screening and testing.
  • Maintaining confidentiality according to HIPAA guidelines; and uses standard precautions and personal protective equipment as needed.
  • Assisting and training co-workers.
  • Documenting services in the medical record.
  • Collaborating with the medical team.
  • Participating as a member of a team to protect the health and safety of the public during emergency situations; secures critical information and maintains morale; and carries out special emergency assignments and works special shifts.
  • Performs other duties as assigned

This position is eligible for a $6,000 hiring bonus in accordance with the County's Hiring and Retention Bonus Program.

Salary is commensurate with experience.

Minimum Qualifications

Bachelor's degree in Nursing; two (2) years of nursing work experience related to assigned department, preferably in public or community health; or equivalent combination of education and experience.

Preferred Qualifications:

  • Experience conducting LTSS evaluations, either community-based or in the hospital setting.
  • Experience with case management.
  • Bilingual or multilingual skills are highly desirable.

Job Contingencies and Special Requirements

  • Possess and maintain an unencumbered Registered Nurse (RN) license to practice in Virginia.
  • Valid driver's license and good driving record (driving records may be reviewed annually for continued qualification).
  • Pass pre-employment background checks to include fingerprinting, NCIC, credit, and DMV checks.
  • Pass TB test within 10 days of hire.
  • Vaccination or proof of vaccination as required for Health Care Personnel by the Virginia Department of Health.
  • Obtain any Department required certifications within established time frames of hire.
  • Complete required OSHA and HIPAA training.
