Hill Haven Nursing & Rehabilitation is a 355 bed skilled nursing facility located in Webster, NY. We provide 24 hour care to our residents and their familes in need of long-term, rehabilitative, transition, or hospice care. Our Certified Nursing Assistants have a passion for providing high quality care with exceptional service & compassion for our residents under the direction of a licensed nurse.

Full Time, Part Time & Per Diem positions available - All Shifts

' REQUIREMENTS:'

Current NYS CNA certification and a High School Diploma or the equivalent is required. Must have demonstrated ability to communicate well both written and verbally with all levels of staff and various populations served.

Must have the ability to build, foster, and maintain positive working relationships.

POSITION SUMMARY::

• Provide and document personal care services such as bed, bath, skin care, hair care, dental care etc. to our residents and patients

• Assist residents and/or patients in and out of bed, bed making and assist with medications or procedures ordinarily self administered as prescribed by physician. • Support the skilled plan of care by carrying out and recording specific nursing care procedures such as applying heat and cold, changing uncomplicated non-sterile dressings, emptying catheter and colostomy drainage bags etc. • Assist the resident and/or patient in plan of rehabilitation by assisting with transfers, walking, and other physical, speech or occupational therapy activities as established by the therapist. • Provide support and encouragement necessary for the residents and/or patients to achieve daily goals. • Document observations and services provided daily. • Report observations and activities to the primary nurse or therapist responsible for the patient and/or resident. • Notify the clinician immediately of any change in the patient/resident condition which may require professional intervention.
